Dr. Joanne Lynn and her Center for Elder Care and Advanced Illness at Altarum Institute have been unusually effective partners in the remarkable work that San Diego County has done to reduce hospital utilization and readmissions through our Community Based Care Transitions Program.

The County's Aging and Independence Services coordinated a coalition that involved all four of the major health systems in the County and a dozen hospitals. Altarum Institute provided expertise and linkages between our community based organization and the hospitals which included coaching, technical and analytic support, access to key federal agencies, and even encouragement. They also assisted us with fine-tuning our measurements for success and helped us advocate with CMS for continuation of the program. We accomplished a great deal and have served 44,000 clients to date. We have been pleased to work with Dr. Lynn and her team who have provided authoritative and catalytic help at every turn.
