Comprehensive care plans for frail elders are essential for improving patient-family-provider communication; for helping patients and families better understand the future they face; for enabling communities to develop and track needs and resources; and for shaping a world in which it is “safe to grow old.”

Altarum Institute's Center for Elder Care and Advanced Illness (CECAI) is leading efforts to prototype such care plans, explore and evaluate their use in clinical settings, and optimize their usefulness in meeting patient and family needs, preferences, and priorities.

The team is also working to have such plans included in the Meaningful Use standards that will shape the future of electronic health records.
