This brief from the Community Living Policy Center at the Heller School for Social Policy and Management at Brandies University outlines a proposal for an innovative program that assists states with improving their home and community-based services (HCBS) infrastructure. Key goals include strengthening housing, transportation, employment, workforce and caregiver supports in communities across the country with initiatives that address social determinants of health (SDOH), so that persons with disabilities and older adults with disabilities are served where they wish to be served. Specifications for the proposal were developed through a workgroup of the Disability and Aging Collaborative.
