Altarum works with federal and state partners to strengthen long-term services and supports (LTSS) systems for older adults, people with disabilities, waiver participants, and people dually eligible for Medicare and Medicaid.
By supporting policy development as well as program design and implementation, we help states deliver coordinated, person-centered services that reflect a person’s needs and goals. We help states improve home- and community-based services and strengthen care coordination. We provide expert support as states integrate Medicare and Medicaid programs to better serve dually eligible populations.

Altarum serves as the evaluation partner for the Community Care Corps (C3), a national initiative funded by the Administration for Community Living. C3 trains volunteers to provide nonmedical support to family caregivers, older adults, and adults with disabilities, helping people maintain their independence at home and reducing the strain on those who care for them.
Across LTSS initiatives, we advance health missions by helping partners design and deliver systems that connect people with the support they need to live independently. The goal in every state engagement is the same: When we're done, the work continues.
