We Specialize in Performance Improvement CME
The Altarum model of CME is proven to result in measurably improved physician performance and patient outcomes.
Performance improvement-based education is a participative skills-building model of CME resulting in vastly improved and measurable patient outcomes. Working with Altarum, the health system, practice, or physician identifies areas in their practice where they see a need for better outcomes in their patients through more appropriate and effective treatment protocols. Areas of improvement are identified, and a curriculum is created to address the perceived practice gap. Altarum assigns clinical professionals to each clinic or practice to guide learners through the program and act as resources on a regular basis. By completing two linked cycles of a data-guided Plan-Do-Study-Act protocol, healthcare professionals learn and apply new strategies and treatment options on their patient population and report the results to measure their progress and return on investment. Eligible participants of our quality improvement programs earn 30-50 Performance Improvement AMA PRA Category 1™ credits and 20-25 MOC Part IV Improvement in Medical Practice credits.
Clinicians can participate in one of four programs Altarum has already developed. The existing quality improvement programs offer both AMA PRA Category 1™ credit and MOC Part IV credit for board certified physicians. These initiatives are aimed at strengthening early identification and reducing the prevalence of 1) childhood dental disease, 2) opioid addiction, 3) depression, and, 4) appropriately identifying and treating patients with trauma. Our programs are designed for clinicians as well as health professionals in non-clinical settings. The SmileConnect® CME helps reduce childhood dental disease by providing quality improvement training for primary care providers with the goal of increasing preventive oral health services. These services include oral health screenings, fluoride varnish applications, and dental home referrals during appropriate well-child visits. This program is available nationally and offers Smiles for Life certification. It is also in alignment with the American Academy of Pediatrics Bright Futures Periodicity Schedule. Our experts have trained over 1,550 primary care providers through SmileConnect CME. The program has yielded clear results. In fact, our clients have reported a 70 to 90 percent increase in the provision of preventive services, which is critical to improving children's oral health.
The Transforming Depression program improves primary care providers' ability to identify and manage patients with depression. Outcomes are aligned with Quality Payment Program quality measures and include program goals focused on increasing depression screening using standardized evidence-based tools and documentation of patient follow-up plans.
While the Transforming Depression program is currently focused in Michigan and Ohio, it can be made available nationally.
The single most significant predictor that an individual will end up in the behavioral health care system is a history of trauma, and the more severe and prolonged the trauma, the more severe are the psychological and physical health consequences. Yet, both behavioral and medical health care providers are often ill equipped to effectively screen and coordinate care for those who may be struggling with complex trauma or other trauma and stressor related symptoms. To address this need, Altarum's continuing education and behavioral health teams collaborated to create a Trauma-Informed Care (TIC) program utilizing a multi-pronged approach to integrate complex trauma screening and coordination of care into traditional TIC within behavioral, pediatric, and family health care settings. We work directly with clinicians and their support staff to adjust their workflow and implementation strategies so that they can appropriately screen and optimally coordinate the care needed for vulnerable patient populations presenting with complex trauma or other trauma and stressor related symptoms.

Greg Makris is Altarum’s medical director for the Great Lakes Practice Transformation Network—part of the Centers for Medicare & Medicaid Services’ Transforming Clinical Practices Initiative. He develops evidence-based guidelines and best practices for practitioners in clinical accuracy and EHR integration. He also administers Altarum's continuing medical education program. Greg holds a medical doctorate from the University of Michigan in Ann Arbor, and is a diplomate of the American Board of Internal Medicine.
Lim manages the accreditation status and reporting for each of the organizations through which Altarum offers continuing education credits. She also serves as the continuing education program manager for the SmileConnect CME, Michigan's Dental Registry, and the Great Lakes Practice Transformation Network CME/MOC programs.
