Altarum Helps to Enhance Data Capacity for Harbor Care Foundation

Altarum has been selected by Harbor Care Foundation, a leading housing organization in Los Angeles County, to provide Training and Technical Assistance aimed at enhancing their data capacity and improving outcomes for vulnerable populations. This engagement marks an important step in Harbor Care’s ongoing efforts to expand its services and ensure better health and housing outcomes for its clients.

Founded in 2019, Harbor Care Foundation plays a critical role in supporting short-term residential facilities that serve as a safety net for homeless individuals recently discharged from hospitals. These individuals, often not stable enough to return to homelessness, receive residential care that includes medical support, medication management, assistance with appointments, and stable housing. Harbor Care’s mission is to ensure the patient's condition is stable and connect them with essential community resources to support their ongoing recovery.

California Advancing and Innovating Medi-Cal (CalAIM) is an initiative to transform the state’s Medicaid program, Medi-Cal, to make it more equitable, coordinated, and person-centered. Harbor Care can now contract directly with Medi-Cal managed care plans for housing-related services. As Harbor Care grows to meet increasing demands, the need for a more robust care coordination platform has become crucial. To address this, Harbor Care has turned to Altarum to work alongside the team to address the unique needs of recuperative care providers, especially with their use of PointClickCare. With an increased understanding that patient care happens outside the four walls of the hospital, the need for software that supports those in recuperative care is growing. Altarum and Harbor Care are bridging this gap to streamline data collection and reporting processes—essential for maintaining compliance with the various funding streams on which the organization relies.

Beyond advancing their software infrastructure, Altarum is also providing Harbor Care with specialized training and technical assistance. With extensive experience in delivering customized training and technical assistance, Altarum is helping Harbor Care’s team to effectively manage data, emphasize best practices, and improve service delivery. This ensures that Harbor Care not only upgrades its technical capabilities but also builds the internal expertise needed to sustain these improvements over time.

As part of this engagement, Altarum conducted a comprehensive issues analysis to identify Harbor Care’s key priorities and is now performing a capabilities assessment to align their operational needs with the available software. This work involves close collaboration with Harbor Care’s leadership, including the CEO, Director of Quality Services, Case Management team, and Quality Assurance Director, to ensure the solutions are tailored to the organization’s unique requirements.

Harbor Care relies on "braided funding," which integrates multiple funding sources to support its clients comprehensively. As dedicated stewards of these resources, Harbor Care is committed to enhancing outcomes for the populations they serve.
